ACC Funded Support Provider
ACC's Home and Community Support Services provide high quality
in-home care for clients to support
rehabilitation and participation in everyday life. Support is
based on the clients' health and safety needs to prevent injury and
harm. ACC will consult with the client about any natural,
family, and community supports that are available to them.
ACC-funded support should complement, rather than replace, these
natural supports.
Home
help is help given to a client for domestic activities
(cleaning, laundry, meal preparation, and associated shopping
activities) in their home, when they cannot perform these
activities because of personal injury. Attendant care
is help with personal care, cognitive tasks of daily living and/or
protecting the client from further injury within their ordinary
environment. Registered
Nurse support is given when the level of service requires the
training and certification of a RN to provide direct clinical care
to the client.
ACC client service staff will carry out a phone assessment with
the client, or arrange for a contracted assessor to visit them, to
determine the amount of support the client needs. Support services
are allocated based on the client's health and safety needs and to
prevent injury or harm. ACC will not fund service requests that are
outside of these parameters (such as daily cleaning).
Clients have the right to choose who will provide their
home support services.
ACC client service staff must offer the client a choice of ACC
Provider Companies to supply the approved support, but the chosen
ACC Provider must have support staff competent to provide the
required services and be able to provide the service within their
contracted regions.
If the client does not express a choice, ACC client service
staff will select an ACC Provider Company from a list of
contracted providers in the client's area. ACC Providers with staff
close to the client's location or who may be able to meet specific
client needs, e.g. cultural requirements, will be considered
first.
