Homecare - How does it work?
Once
you have made contact with your agency the first step will be for
one of their Homecare Consultants to run through a series of
questions. These will help establish the requirements of care
and support services that are to be provided.
You can also request that a member of the agency's team come to
your home and assist you with making an informed choice on the
aspects that you need support with. Geneva Health Care's homecare
services include everything from home help, domestic assistance,
sourcing equipment such as rails in the bathroom, right through to
personal care and complex nursing care.
Once this has been agreed upon the Homecare Consultant will
complete your Individual Service Delivery plan (Care Plan), which
is a detailed document outlining;
- Your details, including emergency and GP contacts
- Health problems (relevant to support)
- Living situation, description of your home environment
(applicable to the services being provided)
- Social / community activities
- Specific spiritual, cultural or individual values or beliefs
that the Carer may need to be aware of
- Preferred support times and related routine
- Agreed services, actions, frequency, and goals to work towards
(if relevant)
- Allocation of care and support workers, including start and
finish dates
- Public holiday arrangements
- Risk management
- Your Geneva Care Consultant details
Once you have approved and signed the Service Delivery plan, the
agency will make contact with the Carers (sometimes referred to as
Support Workers) that are in your local area, and match you with
the most suitable person(s) to provide the agreed support for
you.
At Geneva Health Care we're more than happy to arrange for you
and your family to meet two or three different potential caregivers
until you are comfortable and happy with the Carer. This is all
part of our complimentary service, and at no time are you under any
obligation to proceed, and can cancel at any time.
Once the care package is in place and has begun, we keep in
touch with you on a regular basis to ensure that everything is
going well and you are still happy with your carer and Geneva
Care.
